Elm Terrace Gardens is a not-for-profit, Life Plan Community (also known as Continuing Care Retirement Community) open to people of all faiths.

Residents at Elm Terrace benefit from an active retirement lifestyle full of choice and daily opportunities to be engaged.

From beautiful apartment homes and friendly neighbors with whom to socialize to delicious dining services and a membership to the local YMCA, Elm Terrace offers a lifestyle that helps residents live better!

In fact, two recent studies found that seniors who live in Life Plan Communities are more satisfied in their daily lives and are healthier and happier than those seniors who remain in their own homes

We are seeking a compassionate and dedicated Social Worker to join our skilled nursing team.

Your expertise will help enhance the quality of life for our residents and ensure they receive comprehensive, person-centered care.

- Document all interactions, assessments, and care plans accurately and timely

- Participate in interdisciplinary team meetings to discuss resident progress and care strategies

- Educate residents and families about available resources, rights, and health management strategies

Join our team and become part of a supportive, growth-oriented environment committed to providing exceptional care. We value our staff and offer opportunities for professional development, competitive benefits, and a rewarding work experience dedicated to making a positive impact on residents’ lives.
