We are seeking a full time, permanent Medical Case Manager (HIV/AIDS).  This role will report to the Director of Clinical Services. 

The Medical Case Manager plays a vital role in advancing our mission to provide comprehensive, client-centered care to individuals living with HIV/AIDS.

This position delivers coordinated medical case management services that promote access to quality healthcare, treatment adherence, stable housing, and supportive resources.

Working collaboratively with medical providers, community partners, and internal program staff, the Medical Case Manager conducts assessments, develops individualized service plans, and advocates to reduce barriers to care.

The ideal candidate is compassionate, culturally responsive, detail-oriented, and committed to improving health outcomes and quality of life for people impacted by HIV/AIDS.

 

MAJOR DUTIES AND RESPONSIBILITIES: 

  

  1. Perform a face-to-face comprehensive needs assessment within a specified time frame. Schedule off site/home visits as required.
  2. Develop a service coordination plan for each client, with identified goals and objectives that are agreed upon by the client and MCM. Specifically, goals and objectives to address on-going medical needs.
  3. Provide introduction and enroll in the 340B program to ensure adherence to medication.
  4. Make appropriate referrals to link clients with supportive services, including, but not limited to: transportation, emergency resources, housing, medical care, support groups, counseling and nutritional assistance.
  5. Monitor the effectiveness of services and reassess service needs on an on-going basis.
  6. Facilitate individual counseling and support groups as needed.
  7. Maintain consistent client documentation and reports according to agency policies and professional standards.
  8. Ensure that all client documentation and records are handled in accordance with HIPPA Guidelines.
  9. Submit monthly statistical and time management reports.
  10. Attend on-going training for continuous job knowledge.
  11. Keep up to date with professional standards and trends through continuing education by attending conferences, workshops and seminars.
  12. Participate in Agency’s fund raising and other agency events.
  13. Perform other related duties as assigned.

 

Experience and Skills:

EXPERIENCE and QUALIFICATIONS:

Master's degree in social work or related human service discipline or bachelor's degree in social work or related human service discipline with two (2) years case management and experience. Experience in working with persons living with HIV/AIDs, preferred. 

 

Knowledge, Skills & Abilities:

  • Demonstrated ability to communicate sensitively and clearly, orally and in writing. 
  • Ability to work as a part of a multi-disciplinary team. 
  • Ability to work under pressure. 
  • Willingness to travel throughout the city, including economically depressed areas. 
  • Ability to work autonomously yet responds to supervision. 
  • Superior oral and written communication, and organizational skills. 

 

Physical Demands & Work Conditions:

  • Position requires prolonged periods of sitting, working at a computer, and using standard office equipment.
  • Ability to travel locally to client homes, medical facilities, and community partner sites as required, which may involve walking, standing, and navigating varied environments.
  • Must be able to lift and carry up to 20 pounds occasionally.
  • Requires the ability to communicate effectively with clients, staff, and external partners in person and by phone.
  • May involve irregular hours, including occasional evenings or weekends, to meet client and program needs.
  • Work is performed in a combination of office, community, and client-based settings and may involve exposure to varied environmental conditions.
