Reports to: Supervisor of Housing Medical Case Manager

FLSA Status: Exempt

Position Summary:

Bebashi-Transition to Hope is seeking a skilled Housing Medical Case Manager to join our Direct Services team.

This role involves providing comprehensive case management services to approximately 20 clients with HIV.

The position focuses on helping clients achieve stable housing and viral load suppression, adhering to Housing First principles, and leveraging Philadelphia's housing resources.

 

Key Responsibilities:


Client Management:

• Receive and process referrals from the Division of HIV Health (DHH) Housing Program as assigned by the Supervisor.
• Enter client information into the Case Worthy database and adhere to DHH standards and guidelines for Housing Medical Case Management Services.
• Complete Comprehensive Assessment and Plan (CAP) with all clients at intake and every three months thereafter.
• Assist clients with implementing service care plans aimed at securing permanent, affordable housing and achieving/maintaining viral load suppression.


Support and Coordination:

• Facilitate linkages between service systems to help clients access necessary services.
• Coordinate multiple services, monitor progress, and follow up with clients and service providers.
• Advocate on behalf of clients as needed to ensure they receive appropriate support.


Client Interaction and Documentation:

• Maintain regular contact with clients to monitor health status and follow up on referrals.
• Create, maintain, and update client files regularly, documenting medical visits and viral loads.
• Develop discharge plans outlining next steps once clients secure permanent, affordable housing.


Field and Home Visits:

• Conduct ongoing home visits and accompany clients to medical provider appointments as needed.


Additional Responsibilities:

• Perform other duties as required to support the overall mission and objectives of Bebashi-Transition to Hope.

Experience and Skills:

Education and Experience:

  • Bachelor’s degree in social work, Human Services, Sociology, Psychology, or Nursing. Master’s degree preferred.
  • Expertise in HIV case management and client advocacy.
  • Thorough understanding of medical and psychosocial issues related to HIV infection.
  • Familiarity with Housing First principles and knowledge of Philadelphia housing resources.
  • Demonstrated ability to work effectively with diverse populations, including various gender orientations, and sexual, racial, ethnic, and economic backgrounds.

Flexibility:

  • Ability to work flexible hours, including evenings and weekends.

Language Skills:

  • Fluency in Spanish is a plus.
